INFORMATION ABOUT STATE OF FLORIDA DISBURSEMENT UNIT
Payments required by the final Judgment must be made through the State of Florida Disbursement Unit, notdirectly to the person entitled to the money even if that person so requests.

If this is a new court order the court order is redirecting payments through the Domestic Relations Depository, you should take this payment information form to the Domestic Relations Depository to have your account established so that you can make the first payment. This applies only to the first payment for child support and/or retroactive support for this case. Payments made through the Domestic Relations Depository must be made by case, money order, cashier’s check or certified check. Please see the front of this form to find out where other payments should be made.
Manner of Payment
*There will be a 4% depository fee in addition to your child support order. Failure to pay or include this fee in your payment may result in accumulated arrearages*
A payment may be made by mailing a personal check, a money order (issued by some stores as well as post offices), a cashier’s check, or a certified check (issued by a bank) to the State of Florida Disbursement Unit.
Your payment should be mailed in time to reach the State of Florida Disbursement Unit by the date the payment is due. Your payment should be made payable to State of Florida Disbursement Unit (not to the person entitled to the money); and it should have printed clearly on it the court case number (which is at the upper right hand corner of the Final Judgment), the county of your Court, and the name and social security number of the person required to pay the child support.

IMPORTANT INFORMATION ABOUT MAILING ADDRESS CERTIFICATION
- Your are hereby notified that your are required by FLORIDA STATUTE SECTIONS 61.181(3)(a) and (3)(c) to keep the Court and the State of Florida advised as to your current address at all times. Upon a change, you must filed a notice of change of address with the Clerk of Court and serve a copy by mail upon the State of Florida, Department of Revenue, Child Support enforcement, within three (3) days of the change.
- You are further notified that the mailing of a notice of a court hearing to the address you have provided on the MAILING ADDRESS CERTIFICATION will be considered legally sufficient notice for which a WRIT OF ATTACHMENT ) ARREST WARRANT may be issued.
